A 48 year old male patient with Chronic Kidney Disease secondary to diabetic nephropathy, on Maintenance Hemodialysis


A 48 year old male patient with Chronic Kidney Disease secondary to diabetic nephropathy came for admission for Maintenance Hemodialysis treatment. 

Patient got admitted on 2/9/21 with fever, burning micturition and pedal edema, which progressed to Anasarca. Known case of Hypertension, Diabetes Mellitus type 2 since 2 years and hypothyroidism since 6 months. 
He had 5 sessions of dialysis, 1st session was done on 5/9/21. No CME for readmission. Now admitted under CKD on MHD.

PAST HISTORY
No H/o similar complaints in the past. 
History of HTN and DM-2 since 2 years 
for HTN- Tab. Nicardia 20mg Oral BD 
                 Tab. Metx2 25mg Oral BD 
for DM- Inj. ACTRAPID 8U-8U-8U 

History of Hypothyroidism since 6 months. 
for hypothyroidism- Tab. Thyronom 50 mcg Oral OD 

Not a known case of Asthma, Epilepsy, CVA, CAD. 

Family History- Not significant

PERSONAL HISTORY
Diet- Mixed 
Appetite- Normal
Sleep- Adequate
Bowel and bladder movements- regular 
Addictions- None

GENERAL EXAMINATION 

Pateint is conscious coherent and co-operative, well-oriented to place and time. 
Patient is well nourished and moderately built. 
No Pallor, Icterus, Cyanosis, Clubbing, Lymphadenopathy, 
Vitals- 
Pulse- 
Blood Pressure- 
Respirartory Rate- 
Temperature- afebrile
